Pelvic Guide offers individualized, one-on-one care in a private setting. Self-pay rates are listed below; current package options and pricing are available in Jane. If you are unsure which service to choose, begin with a free consultation.

Payment is due at the time of service. HSA and FSA cards are accepted. Insurance benefits and reimbursement vary by plan and are not guaranteed.

  • Booking opens in Jane under Continuity Wellness. Look for Dr. Iwalani Crush.

    New physical therapy patients may book the 75-minute Initial Pelvic or Physical Therapy Appointment. For sex counseling, choose Sex Counseling Initial Intake Session. If you are unsure which service is right for you, begin with a free 20-minute consultation.

    Current clients should select the follow-up service recommended by Dr. Crush. Pelvic floor therapy and physical therapy follow-ups have different rates; if you are unsure which applies to your care, ask before booking. Book through Jane or use the Client Portal in the site header. If your preferred time is unavailable, join the Jane waitlist.

  • Free consultation — 20 minutes | $0

    Initial pelvic or physical therapy appointment — 75 minutes | $250

    Pelvic floor therapy session — 60 minutes | $175

    Physical therapy session — 60 minutes | $125

    Home physical therapy session — 90 minutes | $200

    Sex counseling initial intake — 60 minutes | $125

    Sex counseling follow-up — 60 minutes | $75

    Women’s health coaching — 60 minutes | $100

    Virtual pelvic floor therapy, physical therapy, sex counseling, and coaching are offered at the corresponding rates shown in Jane. Current insured clients may add non-covered extended time: 15 minutes | $30; 30 minutes | $60; or 45 minutes | $90. Jane is the source of truth for current availability and pricing.

  • Please provide at least 24 hours’ notice to cancel or reschedule. Appointments changed with less than 24 hours’ notice and no-shows are charged $100 to the card on file. This fee is not billable to insurance and must be paid before another visit is booked. Genuine emergencies may be considered at Dr. Crush’s discretion.

    Dr. Crush practices independently within Continuity Wellness. Pelvic Guide appointments follow the cancellation policy provided in your intake forms: at least 24 hours’ notice, with a $100 late-cancellation or no-show fee. The massage cancellation notice on the shared booking page applies to a different service.

  • A valid payment card must remain on file. Outstanding balances are handled according to the signed financial policy. Repeated missed appointments or unpaid balances may result in future appointments being paused or cancelled and the episode of care being closed after reasonable outreach. Extenuating circumstances are considered individually.

  • Pelvic Guide accepts Aetna/Meritain and select Blue Cross Blue Shield plans: Anthem, Federal, Highmark, Horizon, and Independence Personal Choice. Coverage varies by individual plan, and verification of benefits does not guarantee payment. For all other plans, care is self-pay; a superbill can be provided upon request for possible out-of-network reimbursement. Reimbursement is determined by your plan and is not guaranteed.

  • Administrative documentation and advocacy services beyond routine charting and copies of existing medical records—such as customized letters, authorization requests, letters of medical necessity, litigation or disability attestations, and peer-to-peer reviews—are billed at $75 per request. Fees for copies of existing medical records, if any, are handled separately and follow applicable federal and Pennsylvania law. Submit requests through the secure Jane portal.

  • Current treatment package options and pricing are maintained in Jane so you can review the most accurate choices when booking. Packages are available for select self-pay services. If you are unsure which option best fits your plan of care, discuss it with Dr. Crush during your consultation or visit. Package terms are presented in Jane before purchase.
