• Dr. Crush holds Pennsylvania direct-access authorization, so you may begin physical therapy without a referral. Pennsylvania permits up to 30 calendar days of treatment without one. Some insurance plans may still require a referral or authorization for coverage, and we can help you determine what applies.

  • Yes. We accept Aetna/Meritain and select Blue Cross Blue Shield plans: Anthem, Federal, Highmark, Horizon, and Independence Personal Choice. For all other plans, care is self-pay and we can provide a superbill for possible out-of-network reimbursement. Reimbursement is determined by your plan and is not guaranteed.   • Every appointment is one-on-one with Dr. Crush. In-person care takes place in a private treatment room. Her biopsychosocial, trauma-informed approach integrates pelvic and orthopedic physical therapy, cancer rehabilitation, and AASECT-certified sex counseling.

    Physical therapy may combine skilled hands-on treatment, guided movement, and education based on your evaluation, comfort, and goals. Dr. Crush explores how different parts of your body may be connected, explains what she is finding, and develops an individualized plan for what to practice and pay attention to between visits. The balance of treatment is tailored to you and the type of appointment.

    As a cancer survivor, she brings lived experience alongside extensive clinical education, helping her recognize, validate, and thoughtfully treat complex health concerns. Care is collaborative, consent-centered, and tailored to your goals.

    New physical therapy patients: choose “Initial Pelvic or Physical Therapy Appointment,” a 75-minute visit. If you would like to ask questions before scheduling, choose the free 20-minute phone or virtual consultation.

    For sex counseling, choose “Sex Counseling Initial Intake Session.” Returning patients should select the follow-up appointment recommended by Dr. Crush. If you are unsure which service fits, start with a free consultation.

    Dr. Crush practices independently within Continuity Wellness. Pelvic Guide appointments follow the cancellation policy provided in your intake forms: at least 24 hours’ notice, with a $100 late-cancellation or no-show fee. The massage cancellation notice on the shared booking page applies to a different service.

  • No. You do not need an established diagnosis or symptoms you recognize as pelvic-related. Dr. Crush also provides physical therapy for concerns involving your hips, spine, abdomen, movement, and recovery after injury, surgery, or cancer treatment. Your evaluation explores your symptoms and goals, including whether pelvic floor assessment is relevant to your care. If you are unsure where to begin, a free consultation can help.

  • No. Evaluation and treatment can include external hands-on care, movement, breathing, and education. When an internal pelvic floor examination or treatment is clinically relevant, Dr. Crush explains its purpose and asks for your consent.

    You may choose an internal assessment at your initial visit or request one later, at any point during your course of care. Internal assessment may also be revisited on an ongoing basis when clinically relevant and when you choose to proceed. Declining initially does not prevent you from choosing it later, and agreeing once does not commit you to future internal assessment or treatment.

    Consent is ongoing. You can decline, pause, or stop at any time. Dr. Crush honors your bodily autonomy and your decisions throughout care. Your care plan is developed around your needs, comfort, and preferences.
